Form I-485 checklist

FORM I-485 CHECKLIST (2015)

Primary Applicant


Adjustment of Status Application
  1. Form I-485
  2. Cover letter outlining contents (if not concurrently filed with Form I-140; see example below)
  3. A check of $1070 for Applicable Fee includes $85 biometric fee(staple the check to the Form I-485)
  4. Two photos (Write Name and A # on back)
  5. Copy of I-140 Approval Notice or Receipt Notice (if not concurrently filed)
  6. I-693, Medical Exam (must be completed by a USCIS-designated physician and submitted in a sealed envelope)
  7. G-325A, Biographical Information
  8. Employment Verification Letter if not filed concurrently with Form I-140
  9. Copy of official birth certificate
  10. Copy of marriage certificate
  11. Copy of Form I-94, both sides
  12. Copy of passport biographical page and visa stamps
  13. Copy of I-797 Approval Notice(s) for H1b and OPT, and OPT card both sides; If J-1 visa, need to include the Waiver Approval Notice. 
  14. W-2 of latest 3 year (08, 09, 10)
  15. Paycheck stab for March, April and May 2011
  16. (#14 and 15 are optional, but better included. I-20 is also not required, but you can put in especially those with visa stamps if you want)
Employment Authorization Application (EAD)
  1. Form I-765, Application for Employment Authorization
  2. No additional fee required if concurrently filed with I-485 application
  3. Two photos meeting USCIS requirements (Attached to the form)
  4. Copy of I-485 Receipt Notice (if not concurrently filed with I-485)
  5. Copy of Form I-94, both sides
  6. Copy of passport biographical page and visa stamps
  7. Copy of I-797 Approval Notice(s) for H1b and OPT, and OPT card both sides
  8. Copy of old EAD if have,both sides and the approved notice.
Advance Parole Application
1. Form I-131, Application for Travel Document No additional fee required
2. Two photos (Attached to the form)
3. Copy of I-485 Receipt Notice (if not concurrently filed with I-485)
4. Copy of Form I-94, both side
5. Copy of passport biographical page and visa stamps
6. Copy of I-797 Approval Notice(s) for H1b and OPT, and OPT card both sides


Dependent(s)
